Working within the guidelines of the Healthy Opportunities Program through the NC Department of Health and Human Services, the HOP coordinator will assist the Healthy Opportunities Program Manager in overseeing, implementing, and growing the Food Bank’s Healthy Opportunities Program. This position will provide support for incoming referrals to administer Food and Nutrition Case Management, Healthy Food Boxes, Prepared Meals, and Produce Prescriptions through NCCARE360.
This position works Monday - Friday from 8:30 am - 5:00 pm. Three (3) days in the office required.
Responsibilities:
30% - Provides case management for Healthy Opportunities services such as Food & Nutrition Case Management, Healthy Food Box Program, ensuring quality service delivery.
30% Coordinates day-to-day operations of HOP, including quality assurance, member follow-up, and service assessment.
20% Records, maintains and reports accurate data for Food Bank Support Services programming.
10%-Manage inventory of Healthy Food Boxes and other program supplies.
10% Provides administrative and financial coordination and reporting, data collection, impact assessment, and program evaluation for continuous improvement.

Qualifications:
Minimum 1 year of experience working with diverse populations, or in health and human services required.
Bachelor’s degree in public health or related subject matter required or equivalent experience working in health or human services role.
Demonstrated ability to effectively communicate, both verbally and in writing to work effectively with a variety of stakeholders from different socioeconomic and cultural backgrounds.
Bilingual (Spanish/English) preferred.
In depth, demonstrated understanding of Medicaid system and State and Federal support programs.
Knowledge of standard Windows based software, with experience of database creation preferred.
Must have valid NC driver’s license with good driving record.
