Description

General Description / Position Summary

Responsible for facilitating all aspects of the Three Square Center, including managing a team of advocates in the community and building and maintaining strategic healthcare partnership initiatives.

Duties and Responsibilities:

Works to develop the competencies of advocates through training, mentorship, and continuous professional development initiatives.
Provide case management assessments and manage the caseload of applicants and students.
Assists in innovative initiatives such as workforce development or Community Health Worker programming and support new programs to create pathways out of poverty.
Market to new community sites and maintain strong relationships with existing sites.
Develop and initiate plans to correct program issues in collaboration with staff, and follow up with Program Partners regarding concerns about staff, services, clients, or partner sites.
Conduct staff evaluations.
Monitors team goals and assesses efficiency and quality of application services.
Assist with the development of grants and/or grant management related to the Benefit Outreach Programs.
Developing and maintaining the department’s budget.
Overseeing and/or orchestrating the department’s schedule.
Coordinate services with healthcare providers, community agencies, and other stakeholders.
Prepare reports and documentation requires by funding sources, regulatory agencies, and the organization.
Stay informed about current best practices, research, and professional development opportunities.
Ensure confidentiality and privacy of client information in accordance with legal and organizational policies.
Manage and scale Three Square’s healthcare partnership initiative to address food insecurity among people struggling with, or at risk of, food insecurity.
Design additional healthcare partnerships and pilot interventions with private and public healthcare entities to address food security among high-risk groups.
Participate in local coalitions, workgroups, boards, and other groups regarding food insecurity.
Other duties as assigned.
Requirements
Qualifications

Master’s degree in Social Work (MSW) from an accredited institution.
Minimum of 2-3 years of experience in social work, preferably in a case management setting.
Three years of relevant experience in the anti-hunger and/or anti-poverty movement. Additional education may be substituted for experience.
Ability to communicate effectively both orally and in writing.
Ability to manage multiple projects with attention to detail, handle interruptions, maintain focus on tasks, and produce accurate work.
Excellent problem-solving and planning skills.
Ability to establish and maintain effective working relationships with Staff and Program Partners.
Ability to work independently and as part of a team.
Proficiency in case management software, Microsoft Windows and Office environments (Word, Excel, Access, Outlook), and standard office equipment.
Bilingual in English and Spanish preferred.
Safe driving record, current vehicle insurance, and ability to use personal vehicle for Three Square business.
Must be able to pass a criminal background check and drug and alcohol screening.

Work Conditions

Work is typically performed in an office or at a variety of community agencies. The employee will use computers and phones extensively and will drive for work performed in the community. Occasional work outside of normal working hours, such as evenings, as well as periodic travel will be required.
